The Supportive Housing Program assists individuals 18 years or older who are diagnosed with a chronic and persistent mental illness to live in the most independent setting possible in the community. The intent of this program is to increase the community based living options for people with mental illness. These apartments for adults are permanent homes that may be single or shared occupancy.
The goal is to provide affordable housing for these individuals while providing them with supportive case management services, assistance with rent, and individual support services to help them succeed in this setting. These individuals are engaged in their communities in day treatment, school, supported employment, competitive employment or volunteer work.
