Saint Dominic’s Family Services is committed to ensuring that individuals and families seeking services are connected with the most appropriate and least restrictive level of care within the agency. Our programs work collaboratively to facilitate referrals, assess eligibility, support appropriate placement, and ensure that services are provided in accordance with regulatory requirements and program guidelines. The agency also monitors length of stay and discharge planning to promote continuity of care and successful transitions for the individuals and families we serve.
